What is Form IEPF-5?
Form IEPF-5 is the application to the IEPF Authority for a refund — that is, for return of shares or amounts transferred to IEPF. It is filed online on the MCA portal. After submission the portal generates an acknowledgement with a Service Request Number (SRN), which is used to track the claim.
Prepare before you start
- Company name and exact folio number or DP ID / client ID of the original holding
- Number of shares and the years in which dividends were unpaid
- Claimant details exactly as they appear on identity documents
- A working mobile number and email address for OTP verification
- Bank account details of the claimant, with the supporting document the form asks for
- Demat account details for credit of shares
- For legal heirs: details of the original holder, relationship, and entitlement documents
Common causes of queries
- Spelling or name differences between the form, the company records and identity papers
- Wrong folio or number of shares
- Missing or unclear supporting documents
- Documents sent to the company without the acknowledgement or SRN clearly shown
After filing
Print the acknowledgement and keep the SRN safe. Send the documents the instructions specify to the company's Nodal Officer (IEPF) by a trackable method and keep proof. The company verifies and reports to the IEPF Authority, which takes the decision. Watch your registered email for queries.
