Revised Form IEPF-5 (October 2025): What Claimants Should Check Before Filing

By IEPF Assist Editorial Team · Published 16 Oct 2026 · Updated 18 Oct 2026 · 1 min read

MCA substituted Form IEPF-5 with effect from 6 October 2025. Here is what has been reported about the revised form — and why you should confirm every point on the live form.

The Ministry of Corporate Affairs notified the Investor Education and Protection Fund Authority (Accounting, Audit, Transfer and Refund) Amendment Rules, 2025 as G.S.R. 733(E) dated 1 October 2025. It substituted Form IEPF-5, the claim form, with effect from 6 October 2025.

What has been reported about the revised form

Commentary on the notification mentions the following, among other points:

  • OTP verification of the claimant's mobile number and email address
  • Details of up to 15 folios of shares, and up to 15 entries for amounts claimed, with year-wise claim type
  • A statement of the reason for non-receipt or non-encashment
  • Claimant bank details with a copy of a blank cheque leaf
  • For death cases: details of the original holder, the claimant's relationship, nominees and beneficiaries, and documents such as an attested death certificate, succession certificate, NOC from other holders or nominees, indemnity and affidavit
  • Filing by an authorised representative who submits an authority letter

Sources differ. Secondary summaries do not fully agree — for example on whether the company's entitlement letter is still mandatory or has become optional. We have therefore marked IEPF-5 content on this website as needs review. Confirm each requirement on the live form and the IEPF Authority's instructions before you file.

A practical approach

  1. Read the form's own instructions on the MCA portal.
  2. Prepare your details before starting, so that you do not time out.
  3. Keep proof of everything sent to the company's Nodal Officer.
  4. Use the SRN in all correspondence.
